Your First Visit

Your first visit to Everyone's ENT & Sinus Center establishes a vital foundation for our relationship with you. We ask that you arrive 30 minutes prior to your scheduled appointment if you have not filled out the required medical and insurance forms prior to your visit. You may download the needed paperwork here, and bring it completed at your time of visit. Please help us by bringing the following to your first visit:

• Health Insurance Card. You will not be able to be seen without the proper insurance and personal identification.
• Driver's License.
• A list of all medications you are currently taking.
• Copies of any X-rays, CT Scans, labs, sleep studies, allergy testing, etc. previously performed concerning your condition. Do not assume your previous physicians have sent the information.
• Copies of records from your general physician that are related to your condition/problem.
• Your New Patient forms

The length of your initial visit will depend on your condition and any concerns you may have. After examining the records and information that have been supplied to us, we will conduct a thorough examination, followed by a diagnosis and detailed treatment plan. We encourage you to ask questions, and we will be glad to address any concerns you may have.
